Statement in support of an application for divorce or (judicial) separation on the grounds of adultery
WHAT IS FORM D80A USED FOR?
Form D80A is your statement of support on the grounds of adultery for divorce or separation. Once you have filled it in, you must submit it to the court with your form D84 (form to apply for a decree nisi certificate) and a copy of your ex-partner’s form D10 (acknowledgement of service form). You must put the letter ‘A’ in uppercase next to their signature on their completed form D10 to confirm that it is indeed their signature. If your ex-partner’s D10 form has been signed by a solicitor, then there is no need to confirm the signature.

WHAT IS THE FEE TO SEND FORM D80A TO THE COURT AND HOW TO PAY FOR IT?
This form is free to send to the court alongside form D84.
HOW TO COMPLETE FORM D80A AND WHERE TO SEND IT.
- NOTE 1 – The ex-partner who initiated the divorce proceedings and sent in the initial divorce application form D8 to the court is known as the applicant or petitioner. The other ex-partner is known as the respondent. If the applicant has cited adultery as the reason for your divorce in section 6.1 of form D8 (old version of the form – before 06-04-2022), then the person the respondent is having an affair with is known as the co-respondent.
- NOTE 2 – You can complete this form online then print a hard copy or alternatively download it, print it then complete by black pen.
- NOTE 3 – If you find there is not enough space to complete your answers on your form then please use additional sheets of paper to continue on and attach them to your form. Please write your name, address, case number and question number on the top of your additional sheets of paper.
- NOTE 4 – Please remember to keep a copy of your form for yourself outside of the copy you send to the court.
- Form D80A is 3 pages long.
- There are 5 different versions of this form (D80A, D80B, D80C, D80D, D80E). You will need to complete only ONE of them. The form you use will depend upon what grounds for divorce you have stated in your initial divorce application.
- Form D80A – You must complete this statement of support form if the grounds for divorce is adultery.
- Form D80B – You must complete this statement of support form if the grounds for divorce is unreasonable behaviour.
