WHAT ARE FORMS C(PRA1), C(PRA2) AND C(PRA3) USED FOR?
Form C(PRA1) is a parental responsibility agreement witnessed by the court where the biological father should be granted parental responsibility for the child. There are guidance notes at the back of this form to help you complete the process.
Form C(PRA2) is a parental responsibility agreement where you are asking the court to witness an agreement between a child’s parents and a step parent (the spouse or civil partner of one of the parents) that the step parent should have parental responsibility for the child.
Form C(PRA3) is a parental responsibility agreement where you are asking the court to witness an agreement of the acquisition of parental responsibility by a second female parent.

WHAT IS THE FEE TO SEND FORMS C(PRA1), C(PRA2) AND C(PRA3) TO THE COURT AND HOW TO PAY FOR THEM?
There is no fee to send these forms to the court.
HOW TO COMPLETE FORMS C(PRA1), C(PRA2) AND C(PRA3) AND WHERE TO SEND THEM.
NOTE 1 – You can complete this form online and then print a hard copy. Alternatively you can download it, print it and then complete by black pen.
NOTE 2 – Please send 2-3 copies of your forms to the court so they can distribute accordingly. The court will keep one and send one to each party involved in the application.
NOTE 3 – Please keep a copy of your form and any attachments for yourself outside of the ones you send to the court.
FORM C(PRA1)
- Form C(PRA1) is 2 pages long (double sided).
- If you make the C(PRA1) application, it will affect the legal position of the mother and father.
- The father must make a separate application for each child he wants parental responsibility for.
- At the top of page 1 you will be asked to write the full name of the child, gender, date of birth and the date of their 18th birthday. Please do this in the spaces provided.
- Next you will be asked to write the full name and address of the mother and father. Please do so in the spaces provided.
- Then you must take your form C(PRA1) to the local family court or the central family court. Here, a justice of the peace, justice’s clerk, assistant to the justice’s clerk, or a court official who is authorised by a judge to administer oaths, will witness both mother and father sign the form and complete their section on page 1. To do this –
- The mother must take the child’s birth certificate with her to the court.
- The mother will also need to take with her a form of identity, such as a passport, or driving licence showing a photo.
- The father will need to take with him a form of identity, such as a passport, or driving licence showing a photo.
- Once you have signed and had your signature witnessed, you have make 2 copies of this form and take or send them to the central family court with a copy of the child’s full birth certificate. The address is – Central Family Court, First Avenue House, 42-49 High Holborn, London, WC1V 6NP. If you are posting these form(s), please send by recorded delivery.
- The central family court will stamp your form(s), keep the original copy, and send a copy to each parent at the address on the form.
- A parental responsibility agreement can only end –
- When the child reaches 18 years of age.
- If the court makes a court order when a parent who has parental responsibility for the child makes an application.
- If the court makes a court order where the child has made an application.
FORM C(PRA2)
- Form C(PRA2) is 2 pages long (double-sided).
- If you make the C(PRA2) application it will affect the legal position of the parent(s) and the step-parent(s).
- The step-parent(s) must make a separate application for each child they want parental responsibility for.
- At the top of page 1 you will be asked to write the full name of the child, gender, date of birth and the date of their 18th birthday. Please do this in the spaces provided.
- Next you will be asked to write the full name and address of parent A, the other parent with parental responsibility and the step-parent. Please do so in the spaces provided.
- Then you must take your form C(PRA2) to the local family court or the central family court. Here, a justice of the peace, justice’s clerk, assistant to the justice’s clerk, or a court official who is authorised by a judge to administer oaths, will witness all signatures and complete their section on page 1. To do this –
- The parent(s) must take the child’s birth certificate with them to the court to show that the parents are jointly registered on the child’s birth certificate.
- Alongside this, the parents(s) will need to take with them a form of identity, such as a passport, or driving licence showing a photo. They must also bring a marriage certificate showing that both parents were married to each other at the birth of the child, or a court order granting parental responsibility.
- The step-parent(s) will need to take with them a form of identity, such as a passport, or driving licence showing a photo. They must also bring a marriage certificate to prove they are married, or in a civil partnership, with one of the parents of the child.
- Once you have signed and had your signature witnessed, you have to make copies of this form (one for the court and for each parent(s)/step-parent(s)) and take or send them to the central family court with a copy of the child’s full birth certificate.
