WHEN IS YOUR QUESTIONNAIRE AND REQUEST FOR DOCUMENTS ASKED FOR?
Once you or your ex-partner has submitted form A to start the court proceedings for division of assets, finances, and pensions, the court handling this will contact you and your ex-partner by letter. This letter will inform you of the date and time of the first appointment/court hearing called the first directions appointment (FDA). In the same letter, the court will ask both you and your ex-partner to send 5 documents to the court and to each other or each others solicitors by a certain date. One of these 5 documents is YOUR QUESTIONNAIRE AND REQUEST FOR DOCUMENTS. The other 4 documents are form E, statement of issues, chronology, and form G.

WHAT IS YOUR QUESTIONNAIRE AND REQUEST FOR DOCUMENTS USED FOR?
Your questionnaire is a list of questions you would like your ex-partner to answer relating to financial matters. It is also an opportunity to request documents and further information so there is full disclosure of information relating to financial matters. All questions and requests must be relevant to the issues identified in your court case. This will help the judge at the first directions appointment (FDA).
