Forms You Need For Applying For A Conditional Order

 

There are 2 government forms you may need to apply for a conditional order. They are –

BOX 1 – Form D84 is used to apply for a conditional order which means the court believes you are entitled to a divorce. 

BOX 2 – Form FP6 is a certificate of service form used to inform the court that your ex-partner (the respondent person, the other party or their legal representative) has been sent or given (called SERVED in court language) court paperwork (such as a copy of a completed application form) that require them to be informed, respond, comply with court directions or attend court. 

If you are a sole applicant or a joint applicant turned sole applicant applying for a conditional order you will need to send a completed certificate of service form FP6 to the court informing them that your ex-partner has been sent a copy of your form D84 and been given 14 days notice so the court can process your form D84 application for a conditional order.
