WHAT IS FORM EX105 USED FOR?
Form EX105 is the application form used to apply for help with the fees for ordering a transcript of a court hearing. A transcript is a written document of what is said in a court hearing.
The application form used to apply for a court transcript is form EX107. Form EX105 will be sent alongside form EX107.

WHAT IS THE FEE TO SEND FORM EX105 TO THE COURT AND HOW TO PAY FOR IT?
There is no fee to send this form to the court.
HOW TO COMPLETE FORM EX105 AND WHERE TO SEND IT.
- NOTE 1 – You can complete this form online and then print a hard copy. Alternatively you can download it, print it and then complete by black pen.
- NOTE 2 – If you find there is not enough space to complete your answers on your form EX105 then please use additional sheets of paper to continue on and attach them to your form. Please write your name, address and question number on the top of your additional sheets of paper.
- NOTE 3 – Please keep a copy of your form EX105 and all attachments for yourself outside of the ones you send to the family court.
- NOTE 4 – When you submit your completed form EX105 you will need to attach a copy of your appeal form and your form EX107 which is the form used to apply for a transcript.
- Form EX105 is 5 pages long.
- The first box on page 1 will ask you to write down your case number that you are appealing. If you have an appeal number you can write that down also.
- Next you will be asked the date of the court order you are appealing or intend to appeal and the name of the judge whose court order you are appealing. Please give the information in the boxes provided.
- You will then be asked what part of the hearing you want transcribed – whether you want the judgement only, evidence only or the entire proceedings transcribed. You must tick the relevant box.
- Next you will be asked to detail the reasons why you want the evidence or the entire proceedings transcribed if this is the case. Please write your reasons in the box provided.
- Next you will be asked if you have obtained any estimate of the costs of providing a transcript. If the answer is yes then tick the box and attach a copy.
- Now you will be asked for your details – you title, last name, first name, address, contact telephone number, relationship status, number of dependents, and other dependents.
- Next you will be asked if you have made any previous applications for transcripts to be paid by public expense on this case or another case. If the answer is yes you must give details in the box provided.
- You will now be asked your employment status – are you employed, unemployed or retired. Please tick accordingly and answer the questions in the boxes provided. You will need to attach evidence to support your answers.
- Then you will be asked if the property you live in is your own, you have rented it, are lodgings, you own jointly with someone or other. Please tick the relevant box and give details in the box provided. You will now be asked if you or your spouse has an interest in any other property and if yes you must give details in the box provided. You will need to attach evidence to support your answers.
- You will next be asked to write the amount of money you have in the different accounts listed on the form. If you do not have some of the accounts listed please just write ‘NIL’ in the box. You will need to attach evidence to support your answers.
- Now you will be asked about your usual monthly income. There are a number of boxes and you must write the amount in the box that applies to you. Write ‘NIL’ in the box that does not apply to you. Then add the total monthly income from all the boxes and write it in the total box provided. You will need to attach evidence to support your figures.
- You will now be asked to write down the amount of your monthly expenses in the relevant boxes. The boxes that do not apply to you – please write ‘NIL’ in the box. Then add up the total of all the boxes and write the amount in the total box.
- Now you will be asked if you have any debts and if you do you will have to give details in the boxes provided. You will need to attach evidence to support your figures.
- You will now be asked to sign and date a declaration.
- Form EX105 should be returned to the court with form EX107 and a copy of the appeal form which is the application to apply for a transcript.
