Statement of service form
PLEASE NOTE – There are two forms detailed on this website that can be used to inform the court that you have served (given or sent court documents) any other party or witness involved in your court proceedings. These two forms are form FP6 and form FL415. Generally form FL415 can be used when your court case concerns a non-molestation order, occupation order, or forced marriage protection order. Form FP6 can be used for all other proceedings.
WHAT IS FORM FL415 USED FOR?
Form FL415 is a statement of service form used to inform the family court how and when you served a copy of your application form and any other relevant court papers required and to whom did you serve these papers.
Serving court papers on the other party/respondent means that they must receive the court papers that require them to be informed, respond, comply with directions or attend court.
Court papers refer to any documents that are involved in your court case such as your application form.
Depending upon the court case you can either serve the other party/respondent –
- Yourself personally.
- Get someone to do it for you.
- Ask the family court and they will do this for you.
If you are applying to the court for a court order to protect you then you must NOT serve the other party/respondent yourself personally in these cases. Such protection orders are –
- A non-molestation order.
- An occupation order.
- A forced marriage protection order.
If you get someone else to serve the other party/respondent, then you can complete form FL415 and send to the court to inform them that you have arranged service.
If you want the court to serve the other party/respondent you will need to complete form D89 – request for personal service by a court bailiff – and the court will do this for you. After the other party/respondent(s) has been served, the person who has served the other party/respondent must complete a statement of service form FL415 and send it to the court.

WHAT IS THE FEE TO SEND FORM FL415 TO THE COURT AND HOW TO PAY FOR IT?
There is no fee to send this form to the family court.
HOW TO COMPLETE FORM FL415 AND WHERE TO SEND IT.
- NOTE 1 – If you are completing and submitting form FL415 to the court you are known as the applicant. The other party is known as the respondent.
- NOTE 2 – You can complete form FL415 online and then print a hard copy. Alternatively you can download it, print it and then complete by black pen.
- NOTE 3- If you find there is not enough space to complete your answers on your form FL415 then please use additional sheets of paper to continue on and attach them to your form. Please write your name, address and question number on the top of your additional sheets of paper.
- NOTE 4 – Please send a copy of your form FL415 to the court that is hearing your case.
- NOTE 5 – Please keep a copy of your form FL415 and any attachments for yourself outside of the ones you send to the family court.
- Form FL415 is 1 page long.
- At the top of the form you will be asked to write the court case number, the full name of the applicant and respondent and the name of the court where your case is being heard.
- The form has now 3 vertical boxes next to each other. In the first left hand box you will be asked the name and address of the person served. The middle box will ask you to write the means of identifying the person and how, when and where they were served. If service was done by post then you will need to detail the date, time and place of posting. The right hand box will ask you to detail the papers served.
- As stated above if you are an individual applicant you MUST NOT serve the person you are applying for a court order against yourself.
- At the bottom of the form, you must now write your name in the box, sign and date the form.
